De aanmelding voor opname kan door de huisarts, wijkverpleegkundige, ziekenhuis etc. worden gedaan. Ook door de betrokkene zelf of familie mag de aanmelding gedaan worden.<br \/>\nDit geldt zowel voor een definitieve opname als voor respijtzorg. Respijtzorg omvat opname van tijdelijke duur om de familie of mantelzorger te ontlasten.
