Aanmelden thuis<\/h2>\n

Om voor zorg en ondersteuning in de thuissituatie in aanmerking te komen is geen medische indicatie nodig. De aanmelding kan zowel door de huisarts, wijkverpleegkundige, het ziekenhuis, het verzorgings-\/verpleeghuis of een andere zorginstelling worden gedaan. Uiteraard kan ook de betrokkene zelf, of de familie zich rechtstreeks bij ons aanmelden.<\/p>\n
