“Hoe kom ik in aanmerking voor een verblijf?”<\/h4>\n

De aanmelding voor opname \u00a0in het hospice kan door de medisch specialist, huisarts, het ziekenhuis, de thuiszorgorganisaties of de zorginstelling worden gedaan. Maar ook de betrokkene of familie kan direct contact opnemen via onze co\u00f6rdinator. Klik hier voor de contactinformatie<\/a>.
\nVoorwaarde voor opname is een indicatiestelling voor terminale thuiszorg (levensverwachting tot 3 maanden) die via de Friese Wouden Thuiszorg kan worden verkregen van (CIZ , centrum indicatiestelling zorg).<\/p>\n

“Wat zijn de kosten van het verblijf?”<\/h4>\n

Voor het verblijf in het hospice vragen wij een bijdrage van \u20ac 42,50 per dag. Dit is inclusief maaltijden en uw was wordt gedaan.
\nAfhankelijk van uw aanvullende zorgverzekering wordt dit bedrag soms deels of geheel vergoed. Informeer naar de voorwaarden bij uw zorgverzekeraar.<\/p>\n

“Van wie krijg ik medische zorg?”<\/h4>\n

De medische en verpleegkundige zorg in het hospice is in handen van uw eigen huisarts en van De Friese Wouden Thuiszorg. Indien uw eigen huisarts niet beschikbaar is, dan werken wij samen met de huisartsenpraktijk van Medisch Centrum de Poort Drachten.<\/p>\n

“Kan mijn familie af en toe logeren?”<\/h4>\n

Ja, in het hospice is een logeerkamer aanwezig. Ook is het mogelijk dat uw naaste bij u op de slaapkamer kan slapen. Hiervoor plaatsen we een koppelbed waardoor uw bed eenvoudig tot \u00a0een tweepersoons bed gemaakt kan worden. Voor overnachting van familie wordt een vergoeding van \u20ac 10,- per persoon per nacht gevraagd. Wanneer familie gebruik maakt van het ontbijt, vertrouwen wij op een vrijwillige bijdrage.<\/p>\n

“Wat zijn de bezoektijden?”<\/h4>\n

Bezoek is altijd welkom. Dit is afhankelijk van de wensen van de gast.<\/p>\n
